Health promotion and caring: empowerment and capacity-building with families and. The core of the assessment wheel represents the people who make up the community. The solid line surrounding the community core and its subsystems represents its normal line of defense (nld) The dotted lines represent lines of resistance. They are dotted because strengths move in and out. Assessment is a team effort that brings together people with different disciplines, perspectives, agendas, and approaches. Full participation of community members is important at all stages. Working with a diverse team requires sophisticated interpersonal skills. Gaining entry can be challenging: physical entry legitimacy; parameters, psychological entry establish rapport, trust, credibility, know the community as partner model, use the cchna standards of practice as strategies. Cchna standards of practice: health promotion, prevention and health protection, health maintenance, respiration and palliation, professional relationships, capacity building, access and equity, professional responsibility and accountability. Process by which people gain greater control over their health.
