Lecture 15 foot and ankle problems 2. 0. Subungual hematoma: bleeding underneath toe nails, common in distance running and squash secondary to deceleration, toe hits end of toe box and causes disruption in nail/toe bed, also can be acute- stop a weight, step on, etc. Treatment: acute, need to evacuate blood to decrease pressure, can be done with a sterile heated paper clip, use pliers to hold clip, press into nail. Why: pad end of shoes, get bigger shoes, cut toe nails shorter, pad forefoot will push foot against the back of the shoe, adding depth. If bacteria gets under, may cause infection: usually anaerobic bacteria that causes formation of a yellow puss & smell. Epidemiology of ankle sprains: 85% lateral sprain, 10% syndesmosis sprain, 5% medial sprain. Ankle joint functional anatomy: stability comes from, shape of bones mortise, capsule and ligaments static stabilizers, strength of the muscles dynamic stabilizers.
