May be expanded to include other disciplines such as : dieticians, psychologists, neuropsychologists, social workers, recreation therapist. Patient (and family) are encouraged to be an active participant, encouraged to set their own goals, and provided with the education and tools to maximize their functioning. Different patients require different programs of rehabilitation. Same patient will need different programs of rehabilitation at different stages of recovery (functional independence social integration)
