Also known as serous om, or glue ear . 50% children will have ome one month post aom, 20% at 2 months, 10% at 3 months (normal) Protection from nasopharyngeal reflux (bacteria from nose and throat) Kid"s eustachian tubes are shorter, narrower and more horizontal than adults. Dysfunction causes build up of negative pressure in middle ear. Easier for nasopharyngeal contents to reflux into middle ear (reflux may lead to inflammation and effusion) Enhances ability for bacteria to adhere and ascend via eustachian tube. Urti (60% complicated by otitis media from 6 mos to 3 yrs of age) 80% children will have at least one episode of om before school age. Note: symptoms are non-specific (may mean something else) Urgent referral : <6 weeks of age (at higher risk of sepsis and complications) Need to consider additional organisms (e. g. group b streptococci, gram -ve bacilli) Bulging tympanic membrane (indicates build up of pressure and fluid)
