Increase sweating, decrease mobility: nutritional status, assess self-care abilities of clients, factors affecting ability to perform self-care: Illness: strength, age, culture, mental health, cognitive/functional impairment, limited mobility. Incontinence: nursing history to assess, subjective data. Integrity: is the skin in contact: colour, reddened areas, lesions, hair, nails, turgor, nursing diagnosis, example, bathing self-care deficit. Inability to perform personal hygiene r/t: e. g. , weakness, cognitive impairment, pain, limited mobility, etc, deficient knowledge, risk for impaired skin integrity. Interference with free flow of urine: sexual activity, pregnancy, diaphragm or spermicide use, uterine prolapse. Catheter insertion: age related changes, those on antibiotics, those with underlying disease. Internal or external: causes, straining to defecate, pregnancy, sitting/standing for long, obesity, chronic constipation, low fiber diet, heavy lifting, signs/symptoms, pain, bleeding, chronic dull aching pain.
