Critical to assess iv site as per site policy (ahs - every hour) Extravasation: when rough medications cause damage due to not entering veins. Pyrogenesis: infection at the site of the iv, can show as a red or yellow streak going up the arm. Phlebitis: damage to the inside of the vein due to iv mechanical stress or too small of a vein size. Hematoma: leaking of blood under the tissue during insertion, removal or patient movement. Air embolism: no amount of air is safe.
