Bedrest: confinement to a bed for more than 24 hours at a time with a patient lying supine traditionally ordered for patients in a severe condition, pregnancy. Moisture trapping tissue ischemia necrosis decreased tissue regeneration stage 1 pressure ulcer: does not blanch. Nclex: individual cannot alleviate pressure, decreased skin integrity, loss of sensory perception, have devices attached to them. Unstageable ulcer: slough (yellowish-tan) or eschar (brownish black) filling a full thickness injury. Gi reflux (ppis - have a slower onset of action than h2 receptor blockers, prodrug), H2 receptor blockers given first oral care, hydrating the mucosa.
