Bulging of the fontanelles (soft spots in the baby"s skull) Vancomycin + 3rd generation cephalosporin : 2-50 years (greater than 50 adds ampicillin) and. Basilar skull fractures as a result of s. pneumoniae, n. meningitidis etc. Vancomycin + cefepime or ceftazidime: penetrating trauma, post neurosurgery and csf shunt as a result of s. aureus, pseudomonas etc. Prevents the emergence of resistance to individual agents. Inhibits transpeptidase , inhibiting synthesis of the peptidoglycan wall. Increased risk of diarrhea compared with amoxicillin. Can lead to opportunistic yeast infections,increased level of eosinophils. Destroyed by beta lactamase (a prep with clavulanic acid is available) *most options in the penicillin family share very similar features and are not overly specific.
