Patho of motor symptoms: motor symptoms due to damage to extrapyramidal system (regulates motor movement, damage to eps causes dyskinesias (disorders of movement) In pd neurotransmission is disrupted primarily in the striatum: proper functioning of the striatum requires balance between dopamine (inhibitory nt) and acetylcholine (excitatory nt, dopamine release inhibits neurons that release gaba, acetylcholine release excites gaba release. In pd excitatory influence of ach goes unopposed excessive release of gaba: extrapyramidal side effects caused by antipsychotics are caused by bloackade of da receptors may be treated with drugs for pd. Therapeutic goal of medications: provide symptomatic relief (no cure) Increase ability to complete adls: drugs primarily help by improving bradykinesia, gait disturbance, and postural instability. Drugs used: dopaminergic agents: drugs that indirectly or directly active da receptors, more widely used, anticholinergic agents: drugs that block ach (block m receptors in the striatum)
