Geriatric case management: geriatric case management is the process of planning and coordinating care for older. Geriatric assessments: the case manager evaluates the cognitive, behavioural, and emotional status of a client. Insight and judgement: cognition, case managers may also conduct functional assessment, functional assessments provide a measure of the individual"s ability to perform basic activities of daily living (adls), including bathing, dressing, toileting, transferring, and feeding. Instrumental activities of daily living (iadls) such as meal preparation, shopping, house-work, financial management, medication management: case managers completing functional assessments should do so in a home visit this has been interrupted during covid-19. Coordinating services: common needs of this population include, difficulty managing personal care and/or home environment, loneliness, depression, pain, caregiver stress, food security. End of life care planning: case managers may support the client and their family in transition to palliative care or hospice, case managers might provide education around advance care directives.
