Mr. johnson is a 67 year old man who has been admitted for rehabilitation after suffering right cerebrovascular accident (cva). Prior to admission mr. johnson was functioning independently. He lives in a bungalow with his wife and has been married for 36 years. Johnson is currently retired, he worked as a mechanic for 28 years. He has 4 children and 5 grandchildren who lives nearby. Johnson and his wife enjoyed an active lifestyle. They attended bingo twice per week and aquatics three times per week at the local recreation centre nearby. Mr. johnson is admitted for short term rehabilitation at the extended care facility located 10 miles from his home. Mrs. johnson visits mr. johnson daily and would like for him to be moved closer to home. He is on the waiting list at a facility about ten minutes from home. Mr. johnson has a past medical history of asthma, coronary artery disease, hypertension and diabetes.
