Planning is the nurses behaviours in which you set client centered goals, plan outcomes, plan interventions and pick which one that you are going to use. Establish priorities: rank based on urgency or importance, helps you to anticipate and sequence nursing interventions for those clients with multiple health problems. High priorities: address the basic human needs intermediate priorities, non emergent needs of a client low priorities, affect the clients future well being. You elicit responses from the client by setting goals and expected outcomes. They provide clear focus it is a collaborative process so keep your client informed goals of care. Client centered goal: specific and measureable is the clients highest level of health to be reached. Nursing sensitive client outcome: as an individual, family or community behaviours or perception that is measurable along a continuum in response to a nursing intervention guidelines for writing goals and expected outcomes. Client goal or outcome: reflect client responses.
