Obsessions: persistent, intrusive thoughts, urges or images that cause anxiety or distress in most individual, and that the individual tries to ignore, suppress or neutralize. Compulsions: repetitive behaviours or thoughts individual compelled to perform in response to an obsession, or according to rigid rules. Hand-washing checking + rechecking, organizing + ordering, counting, repeating words silently. O/c takes more than 1 hour per day or cause clinically significant impairment. Not due to a substance or medical condition. Disturbance not better explained by symptoms of another mental disorder. 1% annual, 1. 5% lifetime prevalence in us. Usually starts in adolescence, early adulthood (can begin in childhood) 10x more common in ocd than in population generally (denys et al, 2004) 33% of ocd patients have mdd when evaluated (tukeletal, 2002) 20% life comorbidity with bipolar disorder (faravellietal, 2004) They engage in distracting behavior to take mind off fear and reduce anxiety and reinforce the compulsions. Symptom has symbolic link to underlying conflict.
