If physician determines the patient is incapable of consenting: physician must identify and obtain consent from an appropiate sdm. Surrogate = one that takes place of another; substitute. Substitute = one that takes place of another; replacment. Doctor/ other hcp must contact your sdm. Can choose to appoint anyone willing and able to act on your behalf. Except: someone who is paid to provide you with personal care, someone mentally incapable, someone under 16 years of age. It is likely to choose someone close to you. Whom you trust and knows you well. Important that you discuss your intent to name person ahead of time: make sure they are willing to act for you if necessary. Most important question: what would this person want if he/she could decide for him/herself. Members: hc team, dr. , nurse, social worker, chaplain. Where hcp may not know your wishes. Hcp have to follow your wishes, if they know what they are.
