Cause of pain & how it impacts patients functioning & well-being. Who ladder - mild, moderate, severe pain. Intramuscularly: rare, bc of pain associated with it or muscle wasting. Transdermal route: more common in community settings - used for ppl with well managed pain. Need to be aware of medication tolerance - when changing meds, have to remember they will be less tolerant to the new drug. Rectal: suppository; more of a bony pain - tylenol responds better. Majority are good with 4hr dosing, some need continuous infusion. When determining starting dose, need to factor in: Constipation - have bowel protocols - meds/suppositories given on routine basis. Sedation/drowsy - should wear off in 2-5 days - if longer, then likely looking at disease progression (as long as meds are being used protocol) Curing pain is a hard thing to do.
