MEDRADSC 1B03 Lecture Notes - Lecture 15: Supine Position, Thoracic Cavity, Hernia
Document Summary
Def: part of the stomach is elevated & protudes through the opening in the diaphragm into the thoracic cavity. 2 types: *sliding hernia (m/c, rolling or paraesophageal. Especially in supine position, and suddenly get back to normal position. A portion of stomach & ge junction protrudes (supine position) Foods tend to stay in that position and cause irritation. Inflammation: fibrosis & stricture (narrowing, sphincture of lumen) A group of ulcerative disorders that occur in the areas of the ugit that are exposed to acid - pepsin secretion. **damage to mucosal barriers: aspirin or nsaid"s drug or alcohol. Increased acid pepsin secretion: alcohol, caffeine. Decreased blood flow to the area, lead to ischemia. Deep layer involvement hemorrhage (mucosa secrets mucous rich in bicarbonate ions to neutralize the acid) Start from mucosa submucosa other layers. Also damage blood vessels underneath hemorrhage. Occult blood in stool not seen (obvious) Masses of solid material or calculi in the bile.
