Lecture 4: pharmacodynamics- potency, efficacy and therapeutic index. Pharmacodynamics- how drug changes body- relationship between plasma drug concentration and therapeutic response. Quantal effects- frequency distribution curve: yes no pt response, did drug reduce systolic blood pressure, not indicate magnitude of response or maximal effect of drug, poor quality study. Therapeutic index: ratio of drug ld50 or td 50 to ed50, tells u overall safety as related to specific toxicity, median lethal dose is 50. Dose of drug that will be lethal in 50 of test animals: median toxic dose td50- dose that will produce given toxicity in 50% pt, big therapeutic index- lots of safety. Phase 1- few target cells are affected by drug at this dose. Phase 2- linear relationship between amt drug administered and degree of client response: allows to determine therapeutic range. Priority is efficacy- ow good is to achieving max therapeutic response-do(cid:374)"t rlly care about potency.
