He had been intubated in icu and then extubated on same day when transferred on medical unit. Md: apply non-breather o2 mask + fluid bolus 1l ns. But, vs remain the same, so chest x-ray is done, central venous line, an arterial line and a foley catheter inserted. Blood sent + arterial blood gas and culture. Suspect right lower lobe pneumonia + septic state (sepsis) Infection (confirmed or suspected) and systemis inflammatory response syndrome (sirs) criteria: presence of 2 or + of: Wbc count >12,000/ml or <4,000/ml or greater than 10% immature (band) forms. Sepsis associated with organ dysfunction, hypoperfusion or hypotension. Hypoperfusion and perfusion abnormalities may include, but not limited to: lactic acidosis, oliguria or acute alteration in mental status.
