The foundation of safe and effective nursing practice is the ability consistently make good clinical judgments about patient care. Clinical: relating to or conducted in a clinic that involves direct observation of a patient. Interpretations and inferences that influence actions in clinical practice. Important definition: an interpretation or conclusion about a patient"s needs, concerns, or health problems, and/or the decision to take action (or not), use or modify standard approaches, or improvise new ones as deemed appropriate by the patient"s response. Clinical reasoning: process of noticing, interpreting, and responding; reasoning based on how the patient responds. Perspective of decision making using a step by step process (eg. algorithms, decision trees, and guidelines). Example: diagnostic reasoning (analytic approach); considering evidence that supports each diagnosis. If there is a rule that covers the situation, clinical judgment is not required.
