Interpreters do not translate everything patients say, they decide what the doctor needs to know. It"s not only important what is said but also what language it is said in. How accents, dialects, etc. are used to tell a message: speech community, group w/ common rules as to when and where a certain language is used, socialization, process by which people learn the culture of their society. 2: use of silence across speech communities, apache use silence when they don"t know someone, nigeria communities do not resort to silence when they don"t know someone, community of practice.
