What makes hiv so difficult to address is our complex relationship with taboos around sexuality: taboos: carry different meanings cross-culturally. Structural factors = initial focus of hiv prevention campaigns because they were the easiest to target: not true prevention efforts initially focused on individual behaviors, then expanded to consider structural factors often beyond individual control. Hiv denialism (western construction of africa as a continent of disease) 78 countries criminal offence with penalties ranging from whipping to execution: kenya, ghana, nigeria, senegal, zambia, trinidad, jamaica. Laws prohibiting or interpreted by law enforcement or the courts as prohibiting gender non-conformity, defined vaguely + broadly = often cruelly enforced. Countries that criminalize same-sex activity = higher hiv prevalence rates. Different circumstance frame individual behavior: health, safety, tradition. Repressive policies and tactics toward sex workers aggravate rather than solve the health and rights problems they experience.
