Bariatric surgery: gastric bypass, vertical banding gastroplasty and gastric banding (banding better, only in morbidly obese, > 50% of excessive wt at 3 yrs post surgery. Should only be used as part of comprehensive program. No drugs currently approved for use in children and ebm limited. Moa - decreases fat absorption by 30% Only for people with high fat diet. Se"s diarrhoea, flatulence, abdominal cramps, haemorrhoids, fatigue. Effect is dose related wt loss of 8. 5-10% vs placebo of 5. 5-6. 6%, 2-4 kg wt loss after 1 year not great. Dose one 120mg capsule tds (during or up to one hour after each meal) May prevent absorption of fat soluble vits. A, d, e and k need sups. Interactions: anticoagulants (reduce absorption of vit k, cyclosporine (decrease conc. ) Phentermine registered for use as a short term (3mnth) tx. Moa acts directly on hypothalamic satiety centre, augmenting brain catecholamine or serotonin levels to reduce appetite.
