Anatomic mapping: abdominal regions, hypochondriac, epigastric, lumbar, umbilical, iliac, hypogastric. Urinary: frequency, urgency, stress (increase intra-abdominal pressure coughing/laughing) Nursing management: assessment, diagnosis, planning, evaluation. Observe stool for: colour, brown, black, yellow, grey, green or bright red, consistency, pasty, liquid, firm, formed, quantity, odour. The order for abdominal assessment is: inspection, auscultation, percussion, palpation. Body mass index: 20 to 25 = normal, 25 to 29. 9 = overweight, 30+ = obese. Waist circumference and waist to hip ratio: women: waist to hip ratio < 0. 8 = normal, men: waist to hip ratio <1. 0 = normal. Bowel sounds: assess all four quadrants, listen for at least 5 minutes before concluding bowel sounds are absent.
