Section 2 health care providers and services in the south east lhin. Helpful for people who have difficulty accessing primary health care by providing health care and health promotion. Community-directed health programs and services are provided for first nations through family doctors and nurse practitioners. Programs protect and improve health through efforts to eradicate communicable diseases and environmental health hazards. People w/ diabetes can access tools and skills through one-on-one counselling and interaction w/ a team of health care professionals. Resources, referral, and treatment are available to those w/ addictions. Various services are provided to those w/ mental health issues in the community: programs for specific health care needs. Palliative care services are provided for terminally ill patients. Programs involving education and family support are provided for those affected by dementia. Community-based support and knowledge are provided to those who are visually impaired. Offer counseling, communication devices, and education to those who are hearing impaired.
