Bind to dopamine receptors postsynapticly in limbic system and basal ganglia. Strong antagonists of receptors for acetylcholine, norepinephrine & histamine. Anticholinergic related effects (dry mouth, blurry vision, sedation, constipation) Sexual effects on libido, dysfunction, swollen breasts, lactation. Metabolic effects increased appetite; weight gain; elevated blood sugars, cholesterol, triglycerides & bp. Target both the negative and positive symptoms of psychosis. Serotonin antagonists (results in metabolic side effects as well as treatment of negative side effects). Block specific dopamine receptors in the basil ganglia and the limbic system. Close monitoring of wbc, minor eps se, risk of agranulocytosis. Quetiapine (seroquel)- causes drowsiness and dizziness, risk for falls. Olanzapine (zyprexa) significant weight gain & sedation. Asenapine (saphris)- sublingual not used in elderly. Aripiprazole (abilify maintena) usually monthly (can be every 6 or 8 weeks) Aripiprazole lauroxil (aristada) - usually monthly (can be every 6 or 8 weeks) Olanzapine pamoate (zyprexa relprevv) every 2-4 weeks.
