Care guidelines hypertension: american college of cardiology: measure blood pressure at every medical visit, base diagnosis on at least 2 measurements, define hypertension as: Kidney disease > 130/85: stepped care: lifestyle change (weight loss, decrease sodium, increased exercise, decreased alcohol) first followed by medication if lifestyle change does not work. Increase dose and number of medications until blood pressure is controlled. Follow up: 1-2 months until control is achieved then every 3-6 months.
