Medical ethics 3/21/16: marquis how to resolve an ethical dilemma concerning randomized trials . Definition of dilemma: dilemma a choice in which both options are bad, moral dilemma a choice in which both options are morally wrong. Dilemma of clinical trials: give care care for individual patient, which eliminates reliable clinical trials, enroll in trial gives reliable knowledge, but risky and undermines doctor/patient relationship. What if doctor has preference: no solid evidence but doctor has a hunch or suspicion, not always baseless. Evidence other than clinical trials: usually not perfect equipoise, other evidence exists before trials start, basic science, animal studies, anecdotes. Personal opinion: guides actions and advice doctors give to patients, doctors cannot offer clinical trial, doctor will have an opinion about which group (control or experiment) is better for the patient. Competent patient: patient is autonomous, no barriers to autonomy, doctor gives adequate disclosure, risks, benefits, and alternatives, marquis emphasizes alternatives.
