However, individuals with chronic disease and other health challenges may be encountered in these settings, so exercise professionals are urged to be prudent in identifying those who need medical clearance. Should be verbalized to the client, patient, or participant before he/ she participates in a physical fitness assessment or exercise program. He or she should be allowed time to ask questions before signing. Another time to reiterate the purpose/goals of the assessment or program and the risks involved. Usually discuss the emergency measures in place. Must mention that the client, patient or participant may withdraw from the assessment or program at his or her own free will. Each organization or instruction must approve form. If client, patient, or participant is <18 y/o, must have legal guardian or parent sign. Medication use including supplements and drug allergies.
