Accessory motion: performed on patients with limited arom/prom, suspicion of instability, history of trauma. Joint mobilizations: grades, grade 1: small amplitude oscillation at beginning of range. Indications: pain and spasm: grade 2: large amplitude oscillations beginning to mid-range. Indications: pain and spasm: grade 3: large amplitude from mid-range resistance/restriction. Indications: increase rom (stiffness dominant: grade 4: small amplitude at limit of available range/restriction. Indications: increase rom (stiffness dominant: dosage: 30-60 seconds and reassess, repeat 2-3x. Side lying close to the edge of the bed. One hand cupping inferior angle (thumb supporting lateral border, fingers supporting medial border) Other hand over supraspinous fossa (heel of hand) Supine with arm relaxed by their side or across body. Standing at the head of the bed; facing patient. Thumbs over superior surface of clavicle, medial to ac joint. Pain over ac joint with end range movements across body or overhead. Force straight up (anteriorly) through lateral aspect of clavicle.
