Cranial Nerve Cases
1. Your patient is unable to smell anything.
a. CN I – Olfactory
b. Test: pick two distinct smells and test each nostril
2. Your patient has lost the vision in the R eye.
a. CN II – optic (left)
b. Test: visual field
3. Your patient’s L pupil seems to stay dilated all the time.
a. CN III – Oculomotor
b. Test: lateral and vertical gaze, shine light in pupil (PEARL)
4. Your patient cannot feel the L side of his face, in the area above the mouth
a. CN V – Trigeminal (V2)
b. Test: sensation to face – graze area with cotton ball
5. Your patient cannot move his L eye to the L
a. CN VI – Abducens
b. Test: lateral gaze
6. The patient has full, active eye movements (of both eyes) in all directions with no
deficits. This means that CN ____, _____ and _____ are all intact.
a. CN III, CN IV, and CN VI
7. Your patient has lost the ability to move the muscles of his face
a. CN VII – facial
b. Test: make facial expressions; looking for asymmetry
8. Your patient is deaf in his L ear
a. CN VIII – Vestibulocochlear
b. Test: auditory test
9. The speech therapist has reported not to give your patient any thin liquids because his gag
reflex is absent
a. CN IX: Glossopharyngeal
b. Test: gag-reflex or swallowing
10. When you ask your patient to say “Ahhhh” he can’t control the sound and starts to choke
a. CN X – Vagus
b. Test: have them say “ahhh”
11. Your patient has been complaining that everytime he tries to eat solid foods he bites his
tongue.
