Specifies patient"s understanding of health status, discharge planning and health promotion behaviors. Formulates five individualized nursing diagnoses based on the data in the assessment: categorizes each of the five nursing diagnoses as high, me- dium, or low priority. Select two of the individualized nanda-i nursing diagnoses of high priority. Identifies defining characteristics and sufficient substantiating data for each of the two nursing diagnoses. Establishes at least two individualized patient outcomes for each nursing diagnosis. Establishes at least four individualized high priority nursing in- terventions to assist the patient in achieving the identified out- comes. Specifies to what degree each outcome was achieved.
