A 27 year old uniparous woman, gravida 3, was referred to st. luke"s perinatal center for evaluation of a spontaneous twin gestation with possible polycystic kidney disease at 24 weeks gestation period. An ultrasound was performed that revealed a twin gestation with concordant growth. There was bilateral enlargement and increased echogenicity of both kidneys on both twins. The biometric measurements of the kidneys exceeded the 95th percentile for gestational age on both twins. Twin a"s right kidney measured 5. 2 cm and the left measured 5. 1 cm. Twin b"s right kidney measured 4. 2 cm and the left kidney measured. The 95th percentile for renal length at this gestation is 3. 3 cm. The amniotic fluid for both fetuses was reduced, but present. The bladder was visualized on both twins, but each appeared small. The renal parenchyma on both fetuses showed small areas of cystic dilation, but was largely homogenous with increased echogenicity.
