The process by which the clinician integrates three areas of knowledge to arrive at the best plan of action for a client: a clinician must integrate knowledge from several different perspectives: Scientific evidence- scientific evidence regarding the symptoms and treatments for those symptoms. Clinical expertise- their own experience with and knowledge about the symptoms and their treatment. Client perspective- the preferences, perspectives, values, and choices of a fully informed client or client caregiver: ensures clients get the most effective and appropriate treatment, guards against fringe therapies. The systematic progress of gathering info about a client"s background, history, skills, knowledge, perceptions, and feelings. In terms of communications: gather info on language, speech, cognition, feeding, swallowing, voice, fluency, and hearing. It is a multidisciplinary process- involves many professionals who bring their diverse knowledge, skills, and experiences to the assessment.
