Administration of acth or excessive production of acth by pituitary stimulates adrenal cortex to secrete excess cortisol. Cushing"s characterized by weight gain and edema with central trunk and cervical obesity (buffalo hump) and round plethoric face (moon face). Excessive catabolism causes muscle wasting; weakness, thin arms and legs, reduced height, and thing fragile skin with purple abdominal striae, bruising and acne. Exogenous obesity due to excessive caloric intake in which body fat evenly distributed and muscle strength is intact. Hypotension: normotensive adults: <95/60, hypertensive adults: < person"s average reading, but >95/60, children: < expected value for age. Rationale: acute mi, shock, hemorrhage, vasodilation, addision"s disease (hypofunction of adrenal gland), decreased cardiac output, decreased cardiac output, decrease in total blood volume, decrease in peripheral vascular resistance, decrease in aldosterone production. Essential or primary hypertension occurs from no known cause responsible for 95% of cases in adults.
