Not more likely to be involved in error, but if they are, something severe could happen. Warfarin, digoxin, methadone (and all the opioids) a top cause of associated with harm from medication error and hydromorphone is the big one and the other one is insulin. Insulin doesn"t cause a lot of issues, psychotherapeutic agents cause more issues, quite a high number of incidence, cause harm through hypoglycemic but people are monitoring so not as many as we expected. 100 000 americans dying from medical errors that were preventable, causing more deaths than car accidents, breast cancer and aids. In 2004, canadian adverse event study, 360 adverse events in 250 patients, 37% of events preventable. Could lead up to 24 000 canadians dying due to medical errors, since we have 1/10th of the population we expected about 10k people, but it was near double. Concluded that the american data was underestimated and canadian data over estimated.
