Posterior heel pain: achilles tendinitis, achilles bursitis, retrocalcaneal bursitis. Need to decide exactly where the pain is. True tendon pain is usually confined to the tendon itself. Inflammation of the bursa between the tendon and calcaneus. Often due to excessive friction or by wearing shoes that are too tight or large tendon. Can have exostosis (callus) which causes ongoing problems. Heel lift in shoe: will put you into forced plantarflexion gastrocs/soleus muscles shorten: takes the muscles of stretch and ease some of the pressure. Pad: will help if the shoes are too loose if they are too tight, will only make it worse. Achilles stretch via gastrocs and soleus stretch: stretch gastrocs: straight leg, stretch soleus: bend knee isolates the soleus. Achilles is a combined tendon of gastrocnemius and soleus. Tendon has no synovial sheath but is surrounded by paratenon. Only vascular tendons are surrounded by paratenon. Runners have 15x greater risk of rupture and 30x greater risk of tendinosis.
