Past focus less on dying, more on prevention: patients and families were neglected by doctors, modern hospice and palliative care was born from the realization that medicine could contribute during the final part of life (multidisciplinary) Process of dying is longer than ever before. Palliative medicine literature does not include the patients direct voice as it should: main function is to relieve suffering. Acute chronic diseases (more prolonged illness and dependence before death) Most people will have about 2 years of poor health before death. 3 trajectories: cancer, organ system failure, prolonged dwindling or dementia/frailty.
