The milky way has a super massive black hole. There are massive black holes in the center of most galaxies. It is not clear how these super massive galaxies are made. How these massive black holes form and how fast we are still learning. Active black holes are the ones that produce things like the jets. The main question is if their there is gas available to fuel the black hole if no gas then no active black hole. The gas between the stars are really low density. We can study the gas and dust directly but only through radio and infrared radiation. These regions are called molecular clouds and these are where the stars are born.
