Ans control of bladder function: pontine micturition centre (voiding) and l-region (storing) are two nuclei in pons that control bladder function. Their axons innervate skeletal muscle of external sphincter, causing sphincter contraction. Case: a 42 yo woman with ms presents for her follow-up visit. She complains of occasional accidents where she doesn"t quite make it to the bathroom on time. However, she also feels like she has difficulty emptying her bladder. What is going on? (how can she have both incontinence and trouble emptying her bladder?: ms commonly affects spinal cord, including tracts to/from sacral neurons involved in bladder control. Treatment: as seen above: can be very difficult to localize lesions based on symptoms. 1: treatment choice is based on function of bladder; a spastic (overactive) bladder is treated w/anticholinergic medication* to decrease contraction of bladder. Difficulty voiding is treated with alpha blocker to relax internal sphincter and/or use of intermittent catheterization.
