Def: inability to conceive after 12 mo. regular unprotected sexual intercourse. It also takes longer as you get older: this can have to do with fertility, although this might be related to the fact pregnancy(ies) When to investigate and/or treat: after 12 months usually, after 6 months, if obvious problems (eg. oligomenorrhea, testicular surgery, older) Causes: i) male factor, ii) anovulatory, iii) tubal factor, iv) other, v) unexplained note: the causes add up to > 100% because 20% of couples have more than one cause of infertility! Need to do h/p although often the history and physical is normal: hx: previous children, surgery, disease, medication, coital problems, p/e: secondary sexual characteristics, testicular volume, vas deferens and epididymis. Seminal analysis: (who criteria, which keeps on changing!) 20 x 106/ml, 40% motile, 40% normal morphology, Causes: 80-90% have oligo- astheno- teratozo- spermia lifestyle, environment, unknown etiology, vitamin e, c, zn supplements, ?anti-es ?antibiotics.
