It is not complete monopoly but there is a powerful monopoly. From many local occupations representing many social classes to single middle-class professions. Replacing external patronage with collective solidarity and professional authority. Treat entire population or at least most of it rather than small segment. There are all kinds of auxiliaries to the medical profession. It is becoming more dif cult to talk about collective solidarity alone, because now there is government. The medical profession has held a medical authority and solidarity. Rather than have multiple occupations treating small segments of the population depending on social status, wealth, and other factors, the profession as a whole treats the entire population.
