It is important to gather information of psychological aspects in a history to make an accurate diagnosis and management plan: Social aspects: social environment, support- very important to assess how much social/occupational support do they have. Demographic data: name, age, religion, race, marital status, children, educational attainment, employment status, housing. History of presenting complaint: this section of the history needs to cover information regarding when was the patient last well and functional, and information about the first sign of change. Aspects to consider when investigating about change include: Personal history: birth and early development, school history, family history, employment history, sexual and reproductive history. Important to take note of medical conditions that have psychiatric-like" effects such as hypo/hyperthyroidism, brain space-occupying lesions, etc. - psychiatric diagnosis can only be made in the absence of such conditions: previous psychiatric treatments and admissions. Personality: predominant attitudes towards self and others, sustained effort, planning for future, tolerance for frustration, trust/relationships, coping styles, warmth/coldness, psychological mindedness.
