Granuloma (tubercle) induced by m. tuberculosis - low power. Usually enough to contain infection in 90% of infections. Activated macrophages fuse multinucleate giant cells, and epithelioid cells. This response contains the organism in 90% of infections. In 90% of those infected, the delayed type hypersensitivity response controls the infection (latent tuberculosis). These patients have no symptoms and are not infectious. Primary tuberculosis: 5% patients do not control the initial infection and suffer a worsening pneumonia with possible dissemination of the bacterium to other organs, e. g. to the meninges o(cid:396) (cid:373)ultiple o(cid:396)ga(cid:374)s (cid:894)(cid:862)(cid:373)ilia(cid:396)y(cid:863) tb - scattered tubercles around body) Secondary tuberculosis: in those who control the infection, 5 10% experience a reactivation. Secondary tuberculosis: in those who control the infection, 5 10% experience a reactivation of their latent disease (usually within 2yrs) Primary and secondary disease is increased in those with a compromised immune system (extremes of age, hiv)
