Indirect and direct service provision (telephone counselling etc. ) Habilitation and rehabilitation (habilitation develop new skills, rehab regain lost function) Provided in a range of community settings (community health centre etc. ) Chronic disease management copd, diabetes, heart failure. Hospital avoidance and rapid response teams (discharge from hospital, etc. ) Transitional care services (provide support services to get people safely from hospital to home life) Palliative care (terminal care, managing symptoms etc. ) Health coaching model (behaviour change to improve health outcomes) Standford model (focus on goal setting and problem solving skills) What patients do and includes a broad set of attitudes, behaviours and skills that they direct towards managing the impact of the chronic conditions on their lives. Important as it influences patients capacity to self-manage their condition, assist in problem solving, decision-making, utilising resources, forming good health care provider-patient relationships and to help patients take action.
