Blood leak from broken blood vessels interstitial spaces. Bright red or purple, fades to blue, bluish-green, and yellow as contusion heals. Skin could be intact and erythemic (reddened), or the skin may be nonintact with open. If nonintact - ulcer may be superficial or very deep. Wounds are clean, clean-contaminated, contaminated, infected, or colonized. Clean-contaminated: surgically made wound; not infected, but has direct contact with normal flora in respiratory, urinary, or gastrointestinal tract - has more potential to become infected. Contaminated: surgical wound or wound caused by trauma; contaminated by breaking asepsis. Infected: infectious process is established; high numbers of microorganisms and purulent (containing pus) drainage or necrotic (dead) tissue. Classic signs of infection: erythema (redness), increased warmth, edema, pain, odor, drainage. Colonized: high numbers of microorganisms present but without signs of infection. Also called pressure ulcer ,decubitus ulcer, bedsore.
